Acton Town Medical Centre was closed at 18:30 on 24th September 2025 and will NOT be reopening. It will be permanently closed as of Tuesday 24th March 2026.

NHS North West London wrote to you in October 2025 to let you know that we had taken the difficult decision to formally end the contact held with Dr W Moustafa and Dr G Murtaza of Acton Town Medical Centre, effective of 24 September 2025.

In that letter we advised that a short-term Caretaker practice, Crown Street Surgery, would provide continued access to GP services whilst the ICB talked to patients and local stakeholders to help decide the long-term options for the provision of GP services for patients of the practice.

NHS North West London has now reviewed all relevant information, including stakeholder feedback, in order to make a decision about the practice. We are now writing to let you know that a formal decision has been made.

NHS North West London is asking patients to register with alternative local practices of their choice as Acton Town Medical Centre will close on 24 March 2026.

As a current patient of Acton Town Medical Centre, we encourage you to register with another practice of your choice, ideally before 14 March 2026, in order to continue to receive medical care. Even if you do not need to see a doctor immediately we would recommend that you register with another practice as soon as possible so that you can access a GP as and when you need to.

Click the ‘Register online with this GP’ link for your chosen GP practice to complete an online registration form. This webpage also gives information about GP practices, including their opening hours.

If you cannot register online, you can obtain a paper registration form from the reception of your chosen GP practice.
